How to detect overbilling and upcoding in health claims: 6 signals
Fraud, waste and error in health claims raise premiums for everyone. Catching them before payment is far cheaper than recovering money afterwards. Here are six signals any claims team can watch.
1. Prices above the usual
Compare each procedure with the typical local price. A provider whose average bills 40% or more above its peers deserves review.
2. Procedure inconsistent with the diagnosis
An ECG for lower back pain, or surgery for a cold, are inconsistencies a system can flag automatically.
3. Upcoding
Systematically billing the most complex code (for example, high-level visits) when the norm is the mid level. Measure the ratio per provider.
4. Duplicates
The same procedure, patient and date billed twice, sometimes with small variations.
5. Unusual patient churn
Many patients who appear once and vanish can point to a clinic that exists to bill.
6. Doctor shopping
A member seeing many different physicians for the same complaint within a few weeks.
The key is not only detecting but explaining: every alert should show which rule caused it and with what evidence, so the analyst decides quickly and the provider can appeal on facts. An AI tool that cannot explain its decisions creates more work, not less.
Interpolgar Nexus applies these signals to every claim line and keeps the evidence in a tamper-proof log. To see how it would work on a sample of your own data, request a pilot.
